Anyone knows what influences the taxi drivers head?

on my first save file i got the janky dog looking head but on the other two I got the gloomy piano theeth skull

three heads three methods of getting them.
1. going through the taxi man's sleeve
2. Going through the caves and going through the big hole without talking to the npc near the taxi man
3. Talking to the npc near taxi man and then going through the cave.
