Should poison be stackable?
Similar to slay the spire or One Step from Eden

No because all the items are balanced by adding about 10-20% dps. Basic dps is around 200-250 and most poisons give around 20 dps. However, you can stack poison from different items, same as other debuffs.
All it takes for poison sources to make me happy is to refresh the clock.

Not sure if they should refresh the clock. There are lots of interactions with "when you apply a debuff, do X", so how would that work? I think part of them not refreshing is so that you have to consider them in your damage rotation.

The same way "when you gain a buff" works. Which is refresh counts as gaining a buff.

Refreshing would make certain items completely bonkers when it comes to debuffs being reapplied and on-hit effects. That said, you can stack different sources of Poison and DoTs (such as Shock) from different players and different items for a greater effect. As long as the icon has a different color (such as multiple druids using their emerald secondary), it will stack with others.

I wasn't aware two whole items from the same set was a lot. And it is not like the mechanic can't be altered.

There's also one of sniper's secondary upgrades.
I think it wouldn't be too hard to make some kind of internal cooldown system that only allows these "proc on debuff" to occur every x seconds for each status. With a +1 queue as well hopefully.
So if you have say that one item that procs an 8s poison when you use your defensive, if you use it again within 8s, it will automatically reapply once it is over. If you used it twice within that first 8s, it would not reapply again after the second 8s unless you used it once more during that second 8s.

Most games have poison non-stacking, so I can kinda see why for easy of understanding it's not.

Apparently you can stack poison as long as multiple players each take one source and they promise different damage amounts. learned that in a run that gave 3 different poison items and 3 people picking poison :P

Not the kind of stacking we are talking about. That kind of stacking is true for all debuffs, even within the same character. You can have multiple poisons/curses/spark/whatever from different items but not multiple of the same poison from one item. If I have the item that applies poison when I hit with my primary, I'll only ever get 1 stack of that poison.
