The biggest factor in regards to how "hard" it would be is difficulty, to be honest. on harder difficulties, you take more damage which would end up eating through your healing reserves, where as on easier difficulties you could steamroll the soldiers with little issue.
i am aware that there are methods of changing the keybinds via third party programs.
I bet the Sorrow is going to be very hard if doing a Rambo run.
you can avoid the sorrow boss fight if you drown yourself and take the revival pill

Especially now that I found quite a bug (Switch) where I kept being constantly staggered due to some reason or another. Enemies even kept shooting in the air. It was on mountain stage 2/3, shortly before 3/3. I slowly tranq'd two on high alert while 90%+ Camo thanks to the animal print but I was supposedly found despite no one being close. I couldn't find a mention of that bug anywhere so I ended up here...
The problem I found is that the reinforcement mechanic is just annoying, ballistic shields and basically every time I refresh the alert/evasion/caution, a new set is called.
Maybe in the Remaster it will be more fun to go around shooting like in a true FPS (i.e not stationary, dunno if it will have it, only saw the crouch walk) or have a proper TPS (current is with dodgy auto-aim or miss) but atm only the boss fights are the actual 'fun' shooters. Even if Snake sounds like playing Rambo with the LMG.
In any event, I can't recommend it. It's neither particularly fun nor really rewarding. Only silently execute some if you intend to explore the area but going for 'European Extreme' isn't so bad... once one figures out the early Ocelot Unit Fight. The tip I found, to stun-grenade them from under the bed once the door falls, is gold. It is actually quite handy to instantly fail the mission (just choose Continue) instead of enduring the time it takes to either reload the game/save or flee and wait it out.
Going gung ho isn't recommended due to the high damage and extra guards, but nothing stops you from doing it. It's usually wise to stay hidden, and go loud if you get rumbled.
But you'd be amazed how far you can get with a shotgun and an assault rifle, it probably wouldn't be too hard especially if you let Snake heal naturally from his injuries so that his maximum health bar value can increase
