I have the audio out of sync with the game and sanke runs very fast

Limit the framerate to 60fps. If you have an NVIDIA card, you can limit it there in the game profile (you may need to add MGS3 to the profile, but it should be easy to find under recent applications in the “add program” list). I can’t speak for AMD cards though.

I also have an NVIDIA card but I've never had such issues, I wonder why.

AMD here, fixed it by disabling "AMD Fluid Motion Frames"
