Your list pretty much matches what I have and I also have not earned the achievement yet, but I also haven't done the mission mode, captured all wraiths (I think I just need their blossomed variations), complete all the camera/lense upgrades, and complete the game on Nightmare difficulty. I was hoping by finishing some of these off (especially Nightmare difficulty and the blossomed wraiths) that maybe I would get a collectible that wouldn't normally be available on other difficulties and/or without beating the special enemy types -- at least it wouldn't be the first game I've seen do this kind of thing in regards to 100% collectibles.
off topic but you should be able to bloom every ghost that can be bloomed on normal. Chance to bloom increases when you snap with a shutter chance shot and the ghost isn't clipping walls or furniture, but if you wait till nightmare when your camera is likely maxed then some of the easier ghosts probably won't survive even 2 shutter shots on type 7 films even though the difficulty boosts bloom chance, so they end up being harder to bloom for me then.
Ah, I hadn't really caught on to how to trigger the blooming of the ghosts, so that information helps a bunch! But I am indeed currently going through a Nightmare run (and both the camera and flashlight are maxxed -- still need to finish off a lense or two), so I'm probably going to run into the same issue. Kind of wish this blooming mechanic wasn't so wildly random and maybe on Nightmare difficulty everything just defaults to their bloomed-state.
Pressure from Misaki's lens and Explode from Choushiro's flashlight also help, especially explode if it's not upgraded since it does chip damage and you get more chances. Clearing all missions with S rank doesn't add any records or notes sadly, I just did that. The 100% walkthrough on gamefaqs for Wii version tells you when and where all bloomable ghosts can be bloomed; can just follow the guide and save prior to trying to bloom the ones you're missing.
Infinity upgrade from S ranking all missions is redundant at this moment, I don't know if it's a glitch but for now the optimal upgrade from clearing all ghost list automatically makes you not use any film on top of turning all shots into fatal frame shots, so once you get that nightmare mode and s missions become almost a joke.
If you still have difficulty getting high total scores on certain missions, just wait until after nightmare mode clear to get moon lens and upgrade it to max.

Thanks for this! I'm currently on a Nightmare run and haven't got to Chapter 5 yet, so hopefully this will do it for me.
UPDATE: Yep, that was the last one I was missing; got the achievement as soon as I picked up the "Torn Diary". Can't believe I ever missed it (multiple times even) seeing as I got pretty much everything in the area around it in previous playthroughs.
Forgot where notes from someone is but echoes code is chapter 8, the code on 4th floor. Even though young Misaki tells you that in a brief cutscene, you still have to exit out of the code panel operation screen afterwards to photo the panel in order to get that picture.
