You can have 3 rooms full of oxygen and be at 19%. This is very easy to check before you do your first jump. All you had to do was vent one more room until you noticed it fell below 20%.
Always check at the start of every fight whether your opponent has hacking and depower oxygen if they do and power it through the fight as needed, but depowering again before it exceeds 20%. Remember to open the doors of the room that was hacked after the fight.
Aim for red sectors down the line so you can get abandoned sectors to pick up more lanius, for obvious reasons. But even that isn't terribly necessary, your 2 lanius can man pilot and engine, which is all you really NEED.
Warning.
What everyone else said, but note that this achievement has a serious bug in it. It requires you to do it all in one go. Because of some coding error, saving and restarting loses the achievement and it "forgets" you've maintained the oxygen levels, and you won't get the achievement.
So if you want to get this achievement, make sure to set some time aside for doing it.
As others have noted, hacking can nearly instantly lose you this achievement if you cut it close. It slams the doors closed, causing rooms to fill up with oxygen. So if you are cutting it close and get hacked, immediately pause and cut power to oxygen.
Alternately, you can just turn off the clone bay and smother the non-Lanius crew and vent the whole ship. I've never felt quite this ruthless, though. And after all, aren't we the good guys?
But this seems like a job for Backup DNA Bank.
OP seemed interested in completing the achievement, whether or not he wanted it for its own sake.
I actually did this with full crew without the Backup DNA Bank, by keeping two non-Lanius crew in the cockpit and the remaining four in the engine room. One was Envoy and the other Virus, so once I hit Sector 8, I had fully trained crew on the four major systems.
So it is possible to do this and still hit the flagship with full crew, just kind of a PITA.
ETA: also if OP is solely interested in the achievement for unlocking the B layout, the other two achievements are both a lot easier.
I think this was fixed, I'm almost certain I've save&quit a few times and still got the achievement a few times.
