On my first run the only room that had oxygen was the helm, and couple of more rooms after a enemy hacking. I didn't get the achievement. Now I started 100% Lanius ship and turned off my oxygen room completely. We'll see if this is bugged or not later today (assuming I don't get murdered horribly with only 2 crewmen...).
0%<Oxygen<=20%
In Lanius A, the arrangement I found best was:
Lanius crew in Weapons & Shields.
Teleporter, Engines & Bridge with atmosphere, everything else vented.
This keeps you at 19% O2, while still being able to have Mantis boarders (who dash from the vented cloning room into the teleporter when they resurrect), along with a full complement of air-breathing crew packed into the Engines & Bridge.
You lose the ability to man sensors & doors.... but IMO that's a worthwhile trade-off for the ability to board.
I have a hunch: I did quit the game once. Does continue break the achievement..?

Oh, and if you're wondering how come I have so many crewmembers... I was extremely lucky and the 2nd sector was abandoned sector. First shop had 2, then I got one from the translator event and 2nd shop had one!
Basically had a pilot locked in the helm and vented the rest of the ship. Sat at about 6% oxygen the entire game.
From what I've seen in my attempts to earn it, and from what others have been saying, the following conditions have to be met:
1) You have to be BETWEEN 1% and 19%. So you have to be ABOVE 0% but cannot pass 19%. If you hit 0%, you won't get it. Same if you hit 20% or more.
2) You MUST have a non-Lanius crew member on board. Someone has to be on your ship who actually needs air.
3) Your O2 room must be powered. (Though this is generally a given as you need to stay above 0%).
Edit: I got layout C for so many ships yet haven't even got layout B for the Kestrel, should finally show that ship some love.
It is possable it is also due to bad luck with hacking. If you have air in the pilor room and someone hacks a 4 tile room elswhere on your ship that can take you over 20% o2 if you dont work quick.
