What almost certainly happened here is that you cloaked after the missile had crossed your shield barrier. That's where the evasion check happens. Raising your evasion after that point has no effect.
If your shields are down, however, the evasion check happens when the missile reaches the impact point in the target room.
I think the game does this because it's simpler to have a single mechanism for all projectiles, rather than one for lasers and one for missiles.
I think it's also helpful to the player to see that miss or hit ahead of time.
In that case, something happened to drop your evasion below 100%, and you just didn't notice it.
For example, maybe they mind controlled your pilot at the last moment.

I have no idea what you got wrong, but you definitely got something wrong. Maybe they hacked your piloting or engines. Who knows?
For instance, I've noticed that the instant I get lvl 2 shields, all enemies I encounter have lvl 2 or 3. The instant I get lvl 3 shields, all enemies seem to have lvl 3 or 4.
I've fought ships at stars where every pulse sets fire to super important rooms on my ship, while the fires started on their ship are in empty rooms or non essential systems. So I'll constantly have fires starting in my shield room, med bay, O2 room, and weapons room, while fires on the enemy ships start in the sensors room, empty rooms, door control...
It's not nearly as blatant as other games, but it's still way too coincidental to be pure RNG.
It's really not, you're just doing the typical human thing of inventing patterns and meaning from randomness.
We know exactly how enemy ships are generated. It has absolutely nothing to do with the player ship.

Maximum possible evasion is 115%. It isn't possible to reach 120%. You were not paying attention to everything and made an error somewhere.
I occasionally get hit when I expect to be dodging via Cloak. I immediately pause the game and look up by the engine icon in the top left corner that tells me my current evasion. When that says something less than 100%, then I look at the ship to see what I forgot about.
In addition to the common situations already posted, something could have ended your cloaking early. Cloaking can get hacked, or ionized, or if you fire non-beam weapons it will end early. I'm not sure if a Zoltan running through Cloaking might end it early in the same way that happens with Mind Control.
If you have the Backup Battery, if it gets hacked it will steal two power from somewhere, it might come out of Cloaking or Engines. Of if the Backup Battery was running your Cloaking or Engines, they will lose their backup power when the battery goes on cooldown.
