Try to wait until they do their attack and then run and try to attack twice. Then move back or dodge/move around them. I'm currently playing on hard and getting my ass kicked but usually you should be fine if you just wait and watch until they finished their attack, go and attack and then move back and repeat.

Also what's up with having to stomp an enemy 20 odd times when they're down to actually make sure they don't get back up a minute later? Iirc in the original you only had to stomp them a couple of times, maybe i'm not remembering that part correctly though.

Hard is another matter altogether. There were plenty of times when it seemed like the enemies countered with a basically instant, completely undodgeable counterattack, especially the mannequins. My best advice for Hard mode is, if you're not enjoying the game, drop it down to Standard and do Hard on NG+ with the Chainsaw.
