There are many different endings depending on what actions you take through the game.
Correction, he dies in every ending. Because he's more then likely dead before he even enters the town.
No, in that ending Mary reaches out from the backseat and touches him. It's In Water because that's the ending where he looks back there. In any case, James really only accepts responsibility for his actions in the Leave ending. Mary never wanted James to drown himself (obviously) and James doesn't believe in suicide (hence him stopping Angela and his later comments on the subject). Laura represents what is left of Mary and taking care of her is James taking responsibility and not going back to his alcoholic drinking, suicidal ideation, or (in the NG+ endings like Rebirth) worship of the Old Gods.
this isn't even kinda true for most of the endings. even if you accepted the entire game as the after life, that'd mean the game starts *after* he dies. the only endings where he's "shown" to die at the ending in specific are in water and maybe the new vhs tape one. and maybe the ufo raygun kills him instead of knocking him out, lmao.
