It would be interesting to hear from someone who played through quickly if they can get multiple endings the same way.
I then replayed it from the 9 squares save, because I had done something slightly odd the first time around, by exiting from the door unlocked with the scarlet egg, but pushed back on the doors unlocked with the rust egg from the other side to check if that door had also been unlocked. This time I made sure to exit via the scarlet egg door without touching the other one and got the Leave ending.
Now suspicious that it was just RNG, I did it again, making sure to exit via the scarlet egg door again and got the Maria ending.
The In Water ending still haven't figured out how that is obtained exactly but my guess is your actions during the two pyramid heads battle.
I just 100% got both the Leave ending and the Maria ending using the door with the scarlet egg.
I didn't look at the knife at any point. I don't even know how you do that.
I just went to check. It's in the "Personal" items menu and yes, you can inspect it. I'm pretty sure I didn't inspect it but it's not impossible that I somehow clicked on it by accident.
Do you have a video of getting the Leave ending by opening the door you placed the scarlet egg in first? Maybe you did something else that overrode it like waiting to listen to the conversation in the hallway between Mary and James at the hospital, instead of going through the door without listening to the whole conversation.
No video. I ran through that corridor as quickly as I could for both Leave and Maria endings and exited the door before the dialogue had finished in both cases, though I think the corridor is sized so it's near the end of the dialogue at that point.
The only thing I can think of which makes sense is that you get an ending based on how you played (I did get the Water ending the 1st time as well) but it has logic to give you a different ending if you haven't gotten that award yet.
