This is actually just Blooper team having no clue about conveying mental illness in a subtle way.

Is the character depressed? Then they've got to have a sad face and cry.
Angela is a CSA survivor. Eating disorders and insomnia are two symptoms associated with CSA. Angela in the original game does show symptoms of both.

While the huge dark circles and aged appearance are clear signs of her insomnia, she also had more subtle signs of bulimia.

If you look closely at the character model, you could interpret her cheeks as being swollen, but the back of the hands have blemishes caused by self induced vomiting. If you compare the skin on her hands to the other characters, it is very apparent. The game never draws attention to this, and it's really easy to miss.

So as previously noted, cheek swelling is another symptom. When someone with bulimia induces vomiting, it causes the parotid salivary glads to swell.

notice how the check bones are evident in the original vs not in the new one: thats clue #1 for obesity (more meat = less visible bone structure).

also notice the wider body structure in the new version vs the original, in which she is almost "skinny". thats clue #2.

Her upper torso looks smaller in the new version, and her breast size seems reduced, in relation with her original body. Thats clue #3.

she was remade as obese, for political reasons. even the original sketches of the character represent her as "thin", or "fit". she looked "unhealthy", but not as consequence from a possible eating disorder. not every victim of abuse or trauma follows the same "path", so thats also a poor excuse to change her design and appearance.

Why do you think her hands are covered in marks? None of the other characters have those marks on their hands. The hands are from the original game. As seen the the initial post.

Bulimia doesn't mean you are skinny. It's an eating disorder were the patient will binge eat, then purge, either by inducing vomiting or taking laxatives.
One of the symptoms is the swelling of the saliva glands, which causes chimpmunk face.
Have a google, it's interesting stuff.

Famously, Jon Prescott had the disorder for 20 years, and he was a stocky bloke.
