2. In the last location of the game, there ought to have been a UI prompt to tell the player what key to press to use an item. However, there are two issues: (1) The key binding prompt shows a smiley face instead of the actual button (mouse button 4) because I'd rebound it off of mouse button 3. (2) There's a standalone key for "detonator" in the options menu but it looks like it's not used at all in the game.
3. The "Gater House Register" document had disappeared from my Docs list, and I do believe it's the last one I'm missing (25 out of 25) to get the achievement for finding all Docs. I've gone back to the doc's location in the world to find it is no longer there, and I quite certainly have picked up and read it the first time. I finished the game with 24 docs and, from looking at the Steam Guide, it does look like I ought to have them all (except the secret one you pick up from a certain corpse I ended up not having in my game due to story reasons)
In the mean time I will lower the requirement for the Logomaniac achievement by 1 document, so you can load your last save and get the achievement without having to wait.
This workaround will be implemented in the next hotfix patch.
When the patch is ready I'll do a Steam event/update with more details. It shouldn't impact save files but you will need to restart Steam (when it's ready).

Also I have to say first-person controls are weird and need some adjusting; why not let the right stick handle camera rotation and use just the left stick as forward/backward movement?
Additional suggestion: how about a 180° turn single button press in first-person mode?
Controller disconnecting unpauses the game if you already paused (which you probably would if you walked away from the game). Just lost 15-20 mins of progress coz of that one when enemies killed me after it unpaused :(
- in the credits, you can seemingly pause the game with Ps5 controller. It opens a invisible pause menu that it seems couldn't be closed. Causing the credits to loop and never reaching the end. I did this accidentally on my second play through seeing if you can skip credits. This seemed to be game breaking as I couldn't seem to back out of the invisible menu but could hear the sounds of the pause menu.
- On the final boss just after collecting the detonator you can run off the map heading back in the transition section. You can get back on the map but can be difficult.
- On the final boss there is no look back feature when using first person
- The bigger zombies, the one's that make the white smoke cloud when killed. Can reanimate even after being burned with lighter. This happens on reloading and transitioning major areas. They slid around on the floor towards you which made me think it wasn't intentional. Attacking them with flame thrower can make them stand back up and they see to have full health again.
- The hold subtitles seems to be inverted, at least for me: I have to leave it unchecked for the subtitles to wait until I press the button.
