Can someone explain "Expiated" status in detail?

The tooltip explanation confuses me tremendously.

The heal also has diminishing returns and heals religious hero's more and pagan hero's less.

As a bonus, some characters get bonus dmg Vs Expiated targets. The diminishing return of healing is only from more expiates, so don't be scared of using it since you can still heal normally.

Can I avoid healing the enemy by not attacking with that character for one turn? Or is it the "next attack" even if you skip a turn?
