Worth noting is the fact, that 4 months ago they have "improved" (according to them) positional audio, which seems to be in conflict with the opinion of players who have experience of both before and after their latest "fix". I wouldn't hold my breath that they will actually fix this.
The issue seemed to be introduced with the big update with bunker an update or two ago. There are a lot of naysayers on this forum, but I believe they can get it fixed eventually.
In the Quarry it is better, but it isn't an enjoyable experience, and there are too many good games for me to settle with "meh". I would love for them to fix this, because this game is otherwise fun enough, but I can't recommend it to anyone until they get rid of this issue.
I don't get snuck up on much anymore, but if i do its probably on bunker. I can hear the voice lines through certain walls and doors that I probably shouldn't in bunker also. As for the sneaky AI that go quiet, i usually catch their directional footsteps as they close in (personally easier with headphones vs speakers). I seem to get a lot of the directional sound i need in bunker but don't have that at times in quarry, which seems better for you.
It's almost starting to seem to me like the game is giving a larger variance of experiences than usual on different hardware profiles (both audio and graphics-wise).
I'm also using headphones, but I don't think that it should be a problem, since directional audio works fine for me in other games. Are you maybe using Spatial Sound in the sound settings of your audio device?
I am not using any spatial sound settings. I do have an enhancement setting though, loudness equalization.
the reason why the sound is difficult to fix may be because they took a play out the Tarkov play book.
in tarkov it's like there are invisible walls where a wall "should" be...like a large door (think of a garage door) whether it's open or closed the game treats it as it's closed. so in some spots you can be looking at someone moving around in a room but since they're "behind a wall" (or maybe a "deaf spot" is a better way to describe it) you can't hear them
I know it's not an easy fix but they at the very least need to fix the sound of the voices because the jump scares are annoying.
