BUG - guillotine not raising

Hit the second lever in the Ice Bound Mausoleum that was supposed to raise the guillotine to the top but nothing happened. Not sure what I did or if I hit a button but the cutscene of it raising stopped early and when I returned to the guillotine it was only half way up. All levers have been turned and can't be activated again so I'm pretty sure I can't progress the DLC and the save file is cooked.
