Execution Bug

Bug started appearing every time I would try to execute starting in Mother of Mother's. The key to trigger an execution on an enemy does not appear and when you perform the execution, The Penitent One's model disappears while the execution animation goes on. When the animation finishes, his model is still gone and you get soft locked by not being able to move. Menus can still be opened.

This happened to me, I think also in the Mother of Mothers but I don't remember because it was last night. First the PO's model went invisible (including the vines coming from his hand that wrap around some enemies, which made the bell guy's execution interesting). After that I couldn't move and had to quit out. Since then, I can't perform an execution, though I can still kill the stunned enemy and do everything else.

Edit: Right after posting this, I kept playing and was able to do executions again. The first time the prompt appeared was while I was in Blood Pact, but I was in Blood Pact some of the time last night, too, and it wasn't working, so I don't know what the actual determining factor is.

Anyone finding this while searching. Restart the game and the bug should be gone for some reason. The game not a save file.
