The room is located above the elevator that goes up after the "Basilica of Absent Faces" boss fight. You have to jump on the elevator, when it is at top, and then double jump agains't the map limit. You will appear on a room with a lot of easter egg portraits.

I've got 100% of the map with the door behind altar (the one that needs 4 figures), so I don't know what I have made different.

I see in your profile that you haven't learned all remembrances in your weapons. Only with your 40 marks, it's not enough, you need to get all marks that you can find in the treasures box (some of them, are hidden or blocked). Also, some of them contains coins.
By the way, can you take a screenshot for each menu windows for your save and the map? To compare with mine and can see if exists any difference.

Yes, you can get all 100% itens and marks in one run. Actually, I missed one or two secret walls and 1 chalice with mark that I forgot existence, besides the maks that appears on the spot where the horses were.
So, if you coundn't get 100% of the marks (115), you have something missing.
But Im pretty sure that getting 100% of map isn't relate to obtain all the itens on map.
Yes, you can get all remembrances for your weapons in one run, and obviosly, is needed to get the 100% game achievement (My save --> https://imgur.com/Utqykyz. The time was reseted when I started the second save). Only had start another new game to get the "<30 min Afilaor" achievement.
The video and the web that I put, helped me a lot for the revisit and to get all achievements.
In my case, I got all collectables, remembrances, altarpieces, quests (all secondary things) and hidden achievements first. The 100% map achievement was the last, because I don't saw the teleport room in The Severed Tower (was my last room that I discover --> https://imgur.com/Q5ev0Li).
I have saw your map and I don't see any difference with mine, but it doesn't detract from the fact that you have left any hidden place in a zone to discover, because your are missing marks, that some of them are in treasures boxes (I think that some of them was hidden) and others you get if revisited the place where the carriage left to you, of the Coin' quest.

I had the same problem, stuck in 99% and I know that I get everything, Like Blasphemous 1, after playing full game, I played again and, to suffering a little less, I had made an Excel with all data information, or almost all, like a guide, where I have organized. I have the item, location, tips or observation for what is needed to get it, an OK column and a Counter column for the OKs, to keep track of how many or how much did I get.
Imagine that for altarpieces, prayers, rosary beads, HP, MP, etc, etc. In the end, I saw that in the first run of the game I could make all the Archievements. When I finished the A and B ending, I still had the 99% in the Map, but concluded all the others Archievements that one can do in the first run (except the 250 Damage, because the stuid here got the Incense of the Envoys before realizing that I needed one or two of the Altarpieces burned). I read many forums and communitys about the 99% and nothing. I was frustraded. So I did what someone here sayed before, start walking all the game again, with the hidden bell rosary bead thing.
Well, after this "Brief" explanation, I just want to say that I've finded the 1% and I'm posting here the picture of the location in my map.
The place is in Labyrinths of Tides. When you're doing the Regula Side Quest, when you brake the pillar, you don't need to return the same way you came. Right down this place, you have an entrance in the right, with some enemies that finish with a dead end, or what it looks like a dead end. This last wall dead end is a hidden wall. when you hit it, you can climb it and you will be in Regula's cave door.
