Guilt Fragments don't always reduce guilt
Every boss I've done after 3 or so attempts the guilt fragment pick ups no longer do anything but I also don't get any further penalty

This is a feature in B2, dying multiple times will give you permanent guilt even if you pick up the fragment.

Hello everyone,

Thank you for getting in touch with us.

We are sorry to hear you have had a problem with the guilt fragment.

When you die, you will fill 20% of your guilt bar. Recovering the guilt fragment will only fill 18% therefore you will receive a 2% guilt penalty for each death until you remove it using a confessor statue. The 2% will continue to stack with each death until you go to the confessor and clear it.

Is this not supposed to reflect on the fervor bar until certain breakpoints?

Thanks for getting back to us.

Unfortunately, there are no breakpoints. As explained above the 2% guilt that continues to stack can only be wiped by going to the confessor and clearing it, otherwise, it will remain and grow in your bar each time you die.

Okay but like I said, my fervor does not decrease a 2%, visibly, so what determines whether or not the bar shows a decrease in max fervor, if not a breakpoint?

And if it's not displaying it on the bar, then is it actually reduced just without saying it?

The game does use breakpoints for Fervor loss. There are 5 different tiers based on how much Guilt you have.

You can see how much Guilt you have and which tier you're currently in under the small orb on the left side of this screen:

The first screenshot shows the IMMACULATUS tier, which you will be in when your Guilt is 9% or lower. When you are in this tier there is no Fervor loss at all.

The second screenshot shows the AFFLICTUM tier, which you will be in when your Guilt is between 10% - 33%. When you are in this tier you will have -25% Fervor.

There are 3 more tiers above this, each reducing Fervor by 25% more than the previous tier.

Because of it being based on tiers, there are some cases where you can pick up your Guilt Fragment and not have any Fervor restored due to your tier not changing. For example, lets say you have 28% Guilt and pick up your Guilt Fragment, reducing your Guilt back down to 10%. Both 28% and 10% Guilt fall into the AFFLICTUM tier, so both before and after picking up your Guilt Fragment your Fervor will be at -25%. This is where a lot of the confusion comes from with the Guilt system as it's not explained well in game.
