Problem with final health upgrade

I got all the chalices and handed them all in. I also did the blood sacrifice multiple times. I have 300 health now and I keep reading that you're supposed to get another +30 health from doing the blood sacrifices at some point but I have done those several times and I still did not get the additional health. Now the chalice is broken and I don't see how I'm supposed to get this.

I already verified game file integrity, did not help.
