Duplicate ques titems?

So far I've found two Unfinished Lullabies and two Cloths of the Old Woman. In my inventory these don't have quantity markers, so i assume these are pure duplicates for some reason?

There seems to be a bug that causes some items to respawn. The Cloth of the Old Woman is one of them on my save, I can go back and pick it up again and again, but if you already have one in your inventory it should say "already obtained" and give you nothing. Seems like a bug.

Not a game breaking bug, as you only need one of each for their related quests, you will just have some extra clutter in your inventory.
