How do you get an S Rank?

Still have to finish hard, and no timer for the trophies, but I only ever got A ranks. What is the metric that impacts the most? Enemies Killed? Time? Variety in kills?

Also, while I'll give it a try, thanks for not making completing permadeath a trophy. That way I'll give it tries from time to time, but not have to look at the incomplete list.

bro i like your game but why you gotta make it like that, like i want to go fast and complete the level as quick as i can but u make it so i have to go like a turtle so i can get S rank, like i find that pretty dumb since u making a fast paced game
