Long story short - it's crossing limits three times, anywhere on the track, not the one spot.
Maybe because you are that unlucky and exceed track limits always in the same corner. IIRC it only counts when the whole car is outside. so if on other turns your car is still inside and in your unlucky corner not you get a warning.
Yup, all four wheels have to cross the white track line for a driver to get penalised. As long as you've got one wheel on the track, you're golden. 👌🏻
