(spoilers, duh...)
There are also several side quests to discover the dirty truth behind what happened to Boussy and Anais. If you follow through them all, you find that Baron Kimbolt paid mercenaries to attack the convoy of Anais and Boussy and kill everyone there (as he wanted the Temerian throne). However, his plan didn't follow through, since Count Maravel changed the route of the convoy to grab the royal children and sell them to Nilfgaard. But the attack was only hald-successful - Boussy was killed and Anais captured - eventually getting the the hands of the Kaedweni.
With the above knowledge, you can go to John Natalis (Temeria constable in charge until a new king is chosen), and present your evidence. You can accuse either Kimbolt, Maravel or both. If you accuse both, since Natalis doesn't have the manpower to arrest them both he decides to arrest just Maravel.
Now, your main choice in this chapter is whether to help Roche free Anais or to go save Triss.
If you help Roche, you break into the Kaedweni camp and kill everyone (including Dethmold). You have another choice - either return Anais as promised to Radovid, or convince Roche to bring Anais to Natalis. If you go to Radovid, he has a good claim to Temeria. If you go to Natalis, Temeria remains independent and united, and Anais to be its future queen.
If you decide to go save Triss instead, then Roche breaks into the Kaedwni camp himself and kidnaps Anais. He takes her away to protect her in secret until she grows, but in the meantime he's declared an outlaw and a wanted man by all the Northern kingdoms. Geralt breaks into the Niflgaardian camp, kills everyone there and frees Triss. More on the implications below.
If you help Philippa, you and Iorveth sneak her out of the dungeon to her quarters. She leads you to a magic dagger which should break the spell over Saskia, but flees from you.
If you go help Triss, Iorveth rescues Philippa himself, but is terribly wounded as a result. In the prologue you are given the option of saving him from capture by soldiers (Triss can heal him) and retrieve the magic dagger he got from Philippa, in the hopes of sometime in the future using it to free Saskia.
If you haven't saved Triss (i.e. you've helped either Anais or Philippa), then the Nilfgaardian ambassador accuses sorceresses (including Triss) of a conspiracy to kill northern monarchs. All present mages are arrested and there's a witch hunt in the north against all magic users. The Nifgaardian plot to cause chaos in preparation for an invasion succeeds.
If you have saved Triss, there are no Nilfgaardians left to attend the summit. Triss publicly accuses Sile in the murders of Demavend and Foltest. Only the few sorceresses which were part of the conspiracy are hunted, and the new conclave of mages is formally established, and will assign mage advisers to the kings of the north.
Regardless of path taken, the summit is interrupted by the dragon (Saskia) coming to rescue Sile and kill everyone there. You have to fight and defeat the dragon, and then you have the choice of either letting it live (still a pawn of the Lodge of sorceresses) or kill it (remove it from the game). If you've taken Iorveth's path and helped Philppa, you can use her magic dagger to free Saskia from the spell - thus ensuring a truly free realm in Upper Aedirn.
A post credits scene shows the Nilfgaardian army crossing a river, beginning their invasion of the northern kingdoms.
