The only real difference I see is the second picture has a scar above Geralt's right eye brow.
Like 4/5 years ago when Witcher 2 was in development they showed some betas, fans in the forums were pissed off for the new good looks of Geralt saying it wasn't lore friendly, so the devs made him uglier... and that's the face you know today.
Basically that.
I see big differences, first one is more adolscent looking with smaller facial structure.
Second one is...well...30 yr old looking sturdier man.
So, fans always make a fuss whenever Geralt gets more smooth face? I've heard fans already made complaints about his new handsome-bearded face in witcher 3.
Geralt should be good looking to plough nearly every woman he ever encounters. He could be withered looking and old, but still should be basically handsome to get...all the women.
