It's so bad, I try to throw knifes and to switch spells but he does everything much faster and sooner or later he will throw at me something at the same time and I lose too much HP compared to him, if I try to hit him a bit more I bleed alot too...
2. Are you sure you're using your steel sword and not your silver? (silver will do practically no damage against Letho). Also, use strong attacks.
3. What sword do you use? If you're still using the starting sword, it won't help you much in this fight. You should be upgrading your gear throughout the game. In this fight, you should have a weapon which does *at least* 20 max damage. Jagged Blade is best, followed closely by the Superb Cutlass.
4. If you're taking too much damage per hit, do you have a good armor on? (Again, assuming there's no mod causing the issues). You should have the Kayran Carapace armor, or the Hunter's Armor. Remember that you can also upgrade your armor with enhancements (Diamond Armor Reinforcement and Dwarven Enhancement are the best).
5. Drink potions before the fight (at a minimum, Rook and Swallow). You can do this if you tell Iorveth that instead of going directly to Letho you'll meet him there. Then you can go and drink the potions before approaching Letho's location. This also gives you a chance to look around Iorveth's lair (where you fought the Arachas) - you'll find there a better silver sword (won't help against Letho, but will be very good later).
6. Use oil on your steel sword - a whetstone or Hanged Man's Venom will give you 20% buff to damage.
3. Where do you get a sword with 20 max damage ?
The game never tells when I should upgrade my gear, I lost my silver sword and bought a new one, but I thought my long sword was still good enough compared to some other swords cause it's longer so I thought I could hit from more far away...
Yea the solution is to just go to a save before the fight and prepare better.
Always compare the stats of swords you find or are on sale by merchants (or their diagram). When you check out a weapon (or armor, for that matter) in your inventory, it also shows the stats of your current weapon (or armor). Compare them and pick the best one.
*** SPOILERS BELOW ***
The best silver sword in chapter 1 is the Sword of Kaer Morhen (damage 15-24, one rune slot), which you can find at the clearing where you beat the Arachas (and first meet Iorveth).
The best steel sword of chapter 1 is the Jagged Blade (damage 19-25, two rune slots), and you can buy its diagram from the dwarf blacksmith in town. However, to craft it you'll need lots of materials, including a lot of Endrega teeth, so this might take some time to gather.
The second best steel sword in the chapter is the Superb Cutlass (damage 16-22, two rune slots), which you can find in the bandit hideout in the swamp, if you've done the Troll quest and decided to help the Troll and killed Dmitri.
You can find other swords (or their diagrams) which would also be decent. You can even find a Dwarven Axe (damage 10-20) lying about in Einar Gausel's house (across from the smith), which would still be better than the starting sword.
EDIT: I was able to "defeat" him. I went back before the conversation with Iorveth/Letho and drunk 3 potions to boost everything. Then in the fight itself, I had to stay in the corners until he threw magic/bombs at me, so I could dodge. Then I attacked him with IGNI when he had defenses down. It took a while.
It's always the same with the boss fights in TW1-3: They seem to be challenging as long as you don't know how to manage them. But if you find the king's way (or the king's ways) the boss fights get easy enough. They are some kind of RPG riddle.
Letho has unpleasant ranged attacks. So Geralt has to prevent Letho from using them. He should be aggressive, then Letho has limited ressources.
As soon as you get control after the cutscene, Geralt should cast Aard immediately to prevent Letho from casting Quen - and then don't let get him off the hook.
The king's way is "Yrden" - if you prefer short bossfights. (Yrden out of the box is sufficent.) If Letho has only 40 -45% of his LPs the fight is over - but not the dialogue. When Geralt says: "You should have surrendered when you could" he should stop fighting and get into safety until the cutscene begins.
Then you finish the fight in less than half a minute, even in dark mode:
