Report a translation problem
The best armor is the Vran Armor[witcher.wikia.com], which you can only craft if you've completed the Mystic River[witcher.wikia.com] quest in chapters 1 and 2.
However, note that if you're playing a Mage character build, it might be better to not craft it and instead keep the Armor of Ysgith[witcher.wikia.com], which is slighly weaker in some aspects but gives better bonuses to signs.
As for steel swords, the best to my knowledge are Caerme[witcher.wikia.com], the Forgotten Vran Sword[witcher.wikia.com] and Dancer[witcher.wikia.com]. (Don't look just at damage range, but also at critical effect, bonuses and how many rune slots there are).
For silver swords, it's probably Deithwen[witcher.wikia.com] or Addan Deith[witcher.wikia.com].
Of course, if you're playing in Dark difficulty, then the Dark armor/sword sets are the best overall (though very expensive).
