No. You can even play on hard just fine, later on, with little to no issues. Death March is for masochists, though. I don't recommend it.
Not really. Witcher 3 shows if you're capable of mopping the floor with an enemy or get wrecked by it i.e. everything is leveled plus you can always pick your own battles (unless they're scripted, that is).
In TW3 combat is more varied and potions + bombs + signs give tons of options, and I'm comfortable playing on Death March. I was kind of afraid to try TW2 on anything higher than normal, guess I'll try on next playthrough.
The combat is good from the very beginning, kvaldir, but you have to get used to it - and you have to learn some important things.
The monsters have their specific weaknesses. Endregas f.i. are sensitive to Yrden. This sign is of great importance even in boss fights, and it is not necessary to invest a single talent point.

Every beginning is difficult - with all more complex games. So don't worry. The sooner you engage in the game, the more you'll enjoy the battles. The most important basic rule: Don't let yourself be surrounded, backside attacks are dangerous for opponents and for Geralt.
This means: move a lot.

I enjoy Flotsam now, but on my first playthrough, like you, I remember being disoriented in the forests not really sure where to go, etc. Best advice I can give is check your journal frequently as each quest at the top will tell you what the next step is and if you make the quest active by selecting it, the ones that are active (that you can actually progress at that point in time) it will let you know where to go on your map.
Witcher 3 is a lot more open and easier to navigate - the whole game is an open world basically. Witcher 2's maps are pretty linear. Games back when it was made were not as opened up, likely due to hardware and software limitations.
That being said, I like Witcher 2 as much as Witcher 3 as I love its story, locations, and characters. The whole series is just awesome.
