Not frame skips but actual in-engine physics skips, what is causing this problem?
look at this 15 secs gameplay I just recorded:

looking around with the camera is smooth so I'm not dropping frames, but when I walk it's like I'm teleporting around a few feet at a time. It's like the game engine itself is dropping whatever the internal game logic version of frames is. What causes this and how can I fix it?

Probably just net lag. Even if you're playing alone, Extinction is still an online mode on PC.

Extinction is local if played solo, not online. You're connected to the master server for stats, but nothing more. The only CoD titles where even solo matches are on dedicated servers and on those only are Vanguard and probably MWIII as well. Talking about zombies.
"Going online" on MP/Ext just means connecting to the server to sync stats and nothing more.

Verify the game's files and delete all your config files in the player/player2 folder. I'd also try to just create a new Windows user and see if that does anything.
From the video it seems like gun animations are also skipping. As you said, it looks like the tick rate in-game is completely messed up or something.
If above doesn't work you could also try to remove any extra HIDs you've plugged in. Weird controllers like wheels and pedals, macro switches and so on.

I just tried the game myself and it ran just fine.

I appear to have fixed it. I have a 240hz monitor, and I think despite the game having a 240hz option in the settings, they must have ♥♥♥♥♥♥ up when they implemented 240hz support, since the game itself only runs at 91 fps max. I don't know for sure.

However, I appear to have fixed it with the use of 3rd party software that removes the 91 fps cap. Setting the fps limit to 250 with vsync keeping it at 240 appears to have fixed the problem for me. Said software may be against the terms of use of the game but frankly IDGAF since i'm only playing this game for solo extinction. If they want me to have respect for their terms of use they should have released a functioning pc port.

The game's capped to 90/125fps, yes. This was done for stability reasons IIRC as the game runs into issues at higher frame rates. I think it's mostly just an issue with how guns are coded.
