Gacha machine bug or intended?

Two out of three times when I used it nothing happened. Is there a % chance to get 2 upgrades?

It says you get 2 random items right? The two items lying down very close to you. You will see a pickup sign when you get close to them.

Yeah,only now three times out of four they didn't drop or disappeared right after. I think it might be when they drop and you level up at the same time, but not sure.
