Use the Kujiki Bomb and perform a takedown on an ememy.
I'll admit that I don't fully understand how the Kujiki bomb works. I wish I had a better understanding of it. I got right in front of a guy, snapped a Kujiki bomb, then got behind him (he was stunned and wouldn't turn to follow me) and did a silent takedown.
There are probably other methods, such as dropping into a floor grate and then doing a grate takedown.
I'm not sure, but I think he has to actually see you before you use the bomb for it to work? Then sneak around behind him right after and take him out.

You don't have to be seen first and can do it from behind or in front. But with Tempest you need to do a smash takedown (loud silent takedown, that is), but for Vanish a smash takedown won't work. It has to be silent.
What I don't understand is all these videos that show guys using the K-bomb on the run. When I try, he always comes to a dead stop as soon as he throws. Is there a quickfire for it? Because I can't find one.
I think it's left trigger + X on a controller, same as Batman's explosive gel. I'll confirm this if I remember to do so when I am home.
