THE CONDITIONS FOR THE TRUE ENDING HAVE BEEN CONFIRMED.
Rules:
The true ending can only be viewed once. To see it a second time after your first viewing of it, you must manually delete your save file.
You (Maybe?) must have all memories.
You must get a "FLAWLESS!" against every boss except the last one.
Visiting Ghost, or Yuki is NOT needed.
Somewhat related, I am trying to go for the Flawless achievement and just now had a run where I did not get hit at all, including Picayune. However, one that exact same run, the skill triggered on an early wave and the run did not award me the achievement.
This seems definitely buggy, given the description of the skill reads "If you are going to get hit, you will instead use a black hole."
i.e. instead of getting hit.
Hopefully this can be made clearer
Welp, guess I better not use it now even though I liked it.
Got my Flawless yesterday, so lemme try and give some tips
1- Optimize your skill tree, I highly recommend getting 30 points before attempting it (25 level ups + 5 overdrives), you can unselect any healing/on-hit/low hp skills as those will be wasted on the run
Putting any remaining points on dice isn't a bad idea, getting a late game Gambling Statue can give a truly game-breaking amount of levels (plus an easy Yuki fight for extra goodies)
2- Don't underestimate Demon, was by far my biggest run ender, I think its patterns are worse than Picayune honestly, sure I can dodge tem most of the time, but sometimes I get too far away and force it to teleport in a way that traps me, frankly in my winning run I simply chained bombs for invenciblity until it was dead
3- High movement speed (230%~) makes Picayune a joke, pretty much everything becomes dodgeable from simply going to the sides. For the few attacks that might still be trouble you can spare a bomb per phase easily
4- Try to fill your 12 mod slots as soon as possible (less if you can luck some of the greek/aphrodite/whatever statue - the one that grants +5% stats and -1 modslot) as soon as you do all your mod rolls will be exclusively from your already installed items, like banning every other item in the pool, great for consistency and synergy
5- Careful not to buildup Curse, most of my enemy hits were Curseformed fishies, even normal lv 3 fishies ended a handful of my runs, aside from that check when adding new monsters each area so you don't add a sniper or something that can shoot, those are the worst
6- Try to choose weapons based on their boss effectiveness, some of them don't scale up great, spreading out too much to melt them quick, Missile is great for being both strong against bosses and mob control at high size
I appreciate the tips, homie. At this point I'm just waiting to roll a good run. None of the bosses are giving me issues and thus far it's entirely been down to enemies bumping early on. Which, yeah, curses lol
My post was more so to highlight the wording of the Autobomb skill and how it conflicts with people unlocking their True End or this achievement respectively.

I was insanely lucky to have gotten that, I was very strong so really the bosses all melted, even flawless on picayune.
But I see some people in the daily who are going over 25 mortality? What are they doing?
When beating the game the standard way, you're prompted to either go left toward more enemies, or right toward earth.
If you pick Left, you loop, fighting until you die at ever increasing mortality and Peril (Peril being a measure of what % bonus speed and health and spawnrate the enemies have)
I was not certain at the time of posting, but now it's been confirmed that you're allowed to be hit by any phase of Picayune and still get it.
You just need to FLAWLESS! against Bunny, Worm, Biker, and Demon.
