Specific statues seem to add 1% of curse when used. The highest I've ever gotten is just 3 on a double loop run. (50 mortality)
Achievements imply that in addition to Ghost listed above, there's also another secret boss named Yuki. Considering that Yuki is the Japanese word for snow in addition to a name, my gut reaction is that it requires something with the snowglobe or general freezing abilities.
Getting your mortality level below 0 by immediately multi-using an hourglass statue on the first wave.
That'd be a cool secret for the future
I was trying to work out how to get the harbinger achievement "I brought a head." so I brought the severed head of OBJ_DEMON to the final boss. Upon killing Picayune, I got new dialog, and a new ending where I saved her.
Is that the unlock requirement? Or did I just happen to try a weird thing on the exact run that the true ending was scheduled to trigger on?
As for Yuki: I got about 4 rerolls pocketed, went over to a district with a gambler's statue, and grabbed a grenade, and while the fuse was priming, exchanged all of my rerolls on levelups. After the dust settled, the statue was gone, and Yuki appeared.
That was done on the same run as above, so, again, that may or may not be a hidden requirement for the true ending.
Just defeated Yuki. Can confirm that using the grenade powerup while next to the gambling statue does indeed trigger the fight.
Just tried bringing the head into the final boss myself but nothing special happened. Can you elaborate more on what exactly you did?

It's the one that's a human statue with a circle behind it.
It asks rerolls in exchange for level ups.
