Unlocking the second to last character profile in the gallery?

Cleared the game (good and bad ending), collected all the teeth (I think, got all the items), and cleared the "Extra Rumors of Princess Mach" extra content, but the second to last character profile (above Kaoru Hazuki) is still marked as ???. Is it tied to killing off your party members (which I haven't done), or am I missing something else?

There's a "true" ending that's separate from the good/bad endings. AFAIK you can only get it after seeing the good ending with all your Mark Bearer allies alive. Do a chapter select from chapter 6 using a save file with a good ending & the Mark Bearers alive and save Sakamoto when you talk to her in the teacher's room by saying "You mean about Douryou & Kinukawa?" followed by "Convince her." If you convince her, she'll rethink her actions and no longer write a report about you, and you won't encounter her in the forest. After that continue through to the good ending like normal. After you successfully purify the Departed, the clock tower bell will ring. Go back into the clock tower when prompted. This will show you the true ending and allow you to get the final character profile for The Departed

Tried following your instructions but couldn't get the option to return to the clock tower, so had to look up a Japanese walkthrough instead. In chapter 6, in the clock tower Suspensive Act, you have to choose to threaten with the wrench and defend with the crowbar. Didn't think to choose those since it didn't seem to affect the good ending at all.

Wow, I never would have found the true ending if not for this thread. I did think it was strange that Sakamoto and the girls couldn't be saved, and there was that one line where Yashiki wonders whether or not he really saved everyone, but given how many people die in the story, I didn't even think about it

I wouldn't have seen it either without this thread. Since there wasn't a trophy for it I assumed there was just the two endings
