As for the boss, are you playing at 60 fps? I remember the fire serpent boss (Guilen) behaving weirdly at high framerates, could be the same case here. Cap the framerate at 60 fps in the game's configuration tool just to make sure.
This worked, locked frames and refresh at 60 and got it on my first try, had the same problem with Dark Fact not sure why I didn't think of that to start with.
Nightmare is pretty much unforgiving and the level up is exponential.
Don't play Falcom games on Nightmare on the first run, especially not anything they made in the 2000s. They actually remember how to make bosses hard, and do so gleefully. Oath Nightmare might be some of the most fun I've ever had, but trying to learn a new boss on Nightmare is horrible because it just kills you too fast to be able to observe much.
on nightmare/inferno it will track you if you jump too early
