You need to beat nightmare boss rush to get inferno mode, you must have glitched it somehow. I already beat nightmare story mode.
Here's a guide on the bosses that someone else made that'd be pretty helpful.

That said, whatever boss you have great difficulty on Nightmare and what specific problems, feel free to post here. I'm sure that others including myself would be glad to lend a helping hand.
When you're finally prepared to go into Boss Rush Nightmare, don't be afraid to take some breaks every now and then, even if it's in the middle of a boss, it can be a damn ordeal.
In any case, best of luck!
EDIT: One thing I didn't see in the boss guide that'd make Gildias much more bearable; Sit right under his head and just jump and pummel him with air attacks, you might have to reposition yourself and hug him so his bite attack doesn't hit you, but it does wonders in making the fight more quicker.
Attacking his head as he's about to stomp out icicles also interrupts that attack and causes him to go for a bite instead.
I don't know what's up with my game then, but I only got inferno after I beat the nightmare boss rush (just did it yesterday!)
I also unlocked inferno after finishing the story in Hard.
