In my opinion, I doubt he is affected by her newly-diagnosed condition at that point of time since he already knew her back in their middle school days.(and not forgetting that he actually forgot he met her at the carnival) He liked her for her "strangeness"(Asperger Syndrome, a neuro disability), not because of who she is. Then later on he became honest with her... well, I really feel sad for River.. It affected her condition to worsen..
John didn't know what came about the rabbits.. because he had lost his toddler's memories. which includes meeting River for the first time at carnival..(how the rabbits come about)
and he made a stupid comment about platypus too...when he was the one winning it lol.
so the whole paper rabbits and platypus from River is wanting to "wake" John from it..
I have a feeling the reason why he wants to go to the moon or be an astronaut is to show that he is better than his brother... he is pretty much jealous since his mom prefers Joey. and also because he shares the same goal with his brother... all the time.
Nicholas is his middle school classmate whereas Joey died way before during their toddler's years. Since he is on medication which messed up his memories, he probably couldn't remember about Joey until the squirrel's stink comes in.
IMO... River loved John since at the carnival... but John grew to love River overtime..
