Searching for an answer
River's condition makes it difficult for her to communicate her emotions. Johnny can see she's sad but he doesn't know what to do about it. A recurring theme of the game is Johnny not really knowing how to handle River's Asperger Syndrome.

Never know what I should do
River is frustrated that she can't communicate what she's feeling the way everybody else can. When Johnny tells her he has trouble reading her, she doesn't know how to respond.
But, there's the chorus:

Until the stars all fall down
They empty from the sky
But I don't mind
If you're with me, then everything's alright
In the end, it doesn't matter as long as they're together.
Looking at it this way make the scene so, so much sadder.
But yeah, it's just such a lovely song. I didn't cry at the end of the game, but this song did lead to quite teary eyes.
But the scene was still horrible and beautiful.
