Steam will natively pick up a PS4 controller (I know this because it's what I use and I also used to have the same problem as you)
When you're running DS4 you're confusing Steam because its picking up basically a PS4 and an Xbox reading.... Like I said DS4 isn't neccesary (you only need that when playing games that aren't on steam).
Just open steam, plug in your controller, and turn it on (honestly it will probably pick up without you turning it on)
1-Make sure to press the "reset" button on back of your PS4 controller so its not linked with the PS$ (honestly sometimes just unplugging the PS4 helps prevent any issues where your controller wants to connect to that)
2-While the controller is off hold the 'Share" button and the "PS" button on your controller at the same time (it will start flashing a blue light).
3-Open your Bluetooth Settings on your computer and select "Add New Device".
4- Choose the option "Everything Else". when you do your computer will search for a bluetooth device and you'll see the controller pop up saying "Input"
5-Click connect... your controller will connect to your PC and if you have Steam open it will automatically connect.
When your controller connects to steam (either with the cord or with bluetooth) it should have a pop up asking you to setup the controller... go there and select "PS4 Controller" .... Like I said there is no need for DS4 when you're using steam... thats a misconception and just creates more confusion.
Only use DS4 when playing games that aren't on steam
