The only thing that is stealthy is using sneak mode to check for enemies behind walls / doors so that they can't surprise you. Of course only using sneak to get around the map will get you killed since you are at one action point.
Enemies "hear" sounds and weapons like the Galand pistol have silenced modifier which means it will not alert every enemy nearby to rush your ass like with every other regular firearm, but it is unreliable and does not seem to always work.
thank you for the replies. i wish we had a more involved stealth system with harder hitting 1 shot silenced weapons that doesnt just involve using the wheel of pain
i would like it if there was a little bit of a clearer indication of how much noise is generated somehow, while sneaking/walking/running, because i really don't have a clue right now, and i tend to not engage in anything stealthy because of that. It seems like sometimes i can walk up to a guy who has their back turned on me, and they won't turn around until i shoot them. And some other time, they clearly heard me and wait for me to open the door. Maybe a UI element with a numerical value of how much noise you made last turn for instance, or some in-between solution. Or maybe even a portion of the tutorial teaching you how stealth works.

That said, the opposite behaviour is nice too, where by i just start blasting a long hallway, then when the dirties are coming in droves, i set the place on fire and walk away, saves a ton of ammo. Doesn't do well with the Q meter though.
Enemies definitely have a line of sight that can be taken advantage but theres also nothing stopping them from straight up turning around due to them wandering, seeing you and then blasting you with lead.
in early missions I did notice some with "z"s above their head like they are sleeping, but other than that yeah trying to stealth up to someone, especially using melee, is scary as hell when they can turn around any moment while you are completely without cover

There is a bit more to it. Those are related specifically to the mind controlled religious enemies of the Churches and it is directly tied to Quasimorphosis, below 100 Quasimorphosis they sleep and above 100 they wake up, they also wake up by "loud" non silencer weapons
If you use Quasimorphosis reducing items like Alcohol and bring it back below 100 those enemies will go back to sleep if they are otherwise not alert.

There is also no need to get in range with melee sneaking, you can just throw some melee weapons which hit harder than most firearms and ones with accurate throw modifier always seem to hit. You can alternatively carry a silenced pistol either as sidearm or in inventory and switch to it when needed.
