i know with a vest, if the durability goes too low it will start dropping items on the floor.
if you can, make a backup of your profile save folder, and then try loading it up again and search around all the rooms just in case it fell to the floor. (there is still sometimes a small gfx bug which shows a Loot icon on dead enemies, even if there is no loot, so its possible that it might do the same in reverse so check bodies too for nearby dropped items tab - maybe you can find it..
you can eat meats etc just to search around and in any case, once you find it or not, you still have the save you backed up. :)
Hell, the morphine I dropped for the quest item was still on ground where I picked up the quest item at. Tbf, it was exactly the spot where the quest item had been, so maybe that might have caused the eventual error when it reloaded the save?
Either way, I am going to do a full search through the map just in case I missed something. Maybe Pgames is right and I dropped at some point back while doing inventory management, but somehow I find that unlikely since the option to evacuate was at the elevator when I went up the floor.

Welp, I was just about to get the save when that happened (just saw yer comment, replied/edited it and misclicked when I reclicked to the game and boom) >_<
I will just mention that when I loaded the save on the 3rd floor elevator room, for some reason the doctor's jones bag was on the ground where I loaded. Maybe the items have similar values?
(never happened to me just a guess)
But I guess it's not an issue anymore.
(i know that regular items can burn up if you set fire nearby, maybe the devs can check if quest items are flammable too?) maybe they could be set to never burn and be indestructible - or if you burn it by mistake, you fail the mission and extract usually :)
