The mission difficulty gradually ramps up as time passes but a few bad runs can leave you with trainee level operatives with basic gear that will struggle unless you get lucky.
Or they could be referring to the initial issue of a dead operatives starting gear not being replaced as it was at launch.

this is a fun game but it doesnt do well with explaining why any of these things happen, though i understand there needs to be a difficulty curve so the player stays invested, usually though at some point the hell demons or whatever they are will end up controling much of the solar system and at that point it gets a little boring fighting them constantly(as well as endlessly aggravating and sometimes lpain unfun/miserable.
when this occurs usually you just reset the game and start again, alot is randomized so youll likely get to find to item/clone chips, unless you've unlocked everything possible and then you've seen it all sadly.
stick with it, it can be an incredibly frustrating game but once you get used to a few things its usually rewarding, there are gonna be times every now and again where youll die because of a mis click or trash mechanics, unbalanced aspects but it wont be always and when its fun it is a good time for sure.
