How to get the best ending possible? (SPOILERS)
Alright, after killing the Pathfinder, we get the rolling shots of the various places we've visited. Vermund, Battahl, the Volcanic Island Camp, etc. and sometimes we get cutscenes of characters. Doing the quest for Brokkr and Sara, we see them smithing together, doing the cutting quest for Glyndwyr, we see him inspecting the healing Arbor Heart. So that got me wondering, what's the triggers for all of them? I wanna do a playthrough where I get all the credit cutscenes and have my Pawn with max affinity, a true Good Ending feel. Can anyone find out what each of them are?

Without spoiler, do all quest correcty, some NPC required to died you need to revive it afterwards, maxed your affinity with pawn, if you did all of this, most of end screen will be filled by NPC that you helped.
No need to give gifts to Rivage Elder. He will still be there in the true ending. The most visited NPC you gifted and talked to will show in the ending scene, the ending will tell you that literally. "Your Friend".

The only NPC that must be revived is that guy blocking your way in the end and hired to guard Vermundian noble earlier.
You can save furry queen by doing assasin quest

Funnily enough, I accidentally threw Raghnall off the bridge, but when I beat the game, I got his cutscene regardless, and he was alive in Battahl

A lot of people have the same reaction as you, including streamer. He must be alive, meaning he's part if the scripted story. So he will still be there even if player kill him.

At OP - I was just wondering this myself, doesn't seem like there is a consolidated list of cutscenes based on the characters you helped (best outcomes only) - offhand I have my list here (I missed probably 2 of them). List and spoilers below:

I forget the exact order -
1. Sven becomes the ruler of Vernworth / Vermund.
2. Doireann is seen planting a new Arborheart tree cutting, with Glyndwyr and Taliesinn behind.
3. Wilhelmina is seen burning letters by a campfire, probably related to Allard.
4. Hugo is seen sitting outside with his daggers, he appears to leave them on the ground, leaving his life of thievery behind.
5. Beren is seen training new recruits.
6. Raghnall survives your duel and has a cutscene.
7. Sara and Brokker are smithing together in their forge at Battahl.

The ones I probably missed were relating to Ulrika (didn't finish the full story arc of Melve) and Nadinia (missed the last quest in Vernworth night time).

Thanks for the screens, I might try to get those on my current play through (NG+2 right now).

Also I noticed something in the True Ending. In the first True Ending, my Main Pawn became human for a moment and expressed his gratitude. In the second run of the True Ending, he became more aggressive and had different lines, he also did not become human and remained in dragonplague form.

First True Ending:

Second True Ending:

Any idea if this changes on NG+3?

It depends on your relations with pawn. If your relations are high it becomes human, if relations are low it stays in dragon form.

If your pawn gets red it means relations are high (humans also smile, while bestmen make duckface)

Best ending possible? You discover that the guy in the hands of the dragon is ugly and can't possibly be your beloved, so you refuse to fight him and become a normal sovereign.
