For what I saw, you always need the mask, additionally.

If you kill Bermudo, let him go, or simply don't complete the quest, Brant will buy a pass from a merchant for you, but it's a Beastren-only pass. That makes you require the mask if you are human, and you won't be granted access if you aren't wearing it.

This is the answer. The outcome of The Arisen's Shadow determines whether you get a permit for everyone or one just for beastren.

Hmm, I have the good permit, but the guard gets snotty with me every time I pass through the gate. Even if I'm coming from inside Battahl, he'll ask to see the permit.
(it also includes the Battahl Residence Permit in the selection window, so that might also be an option if you have the Mask version of the entry permit.)

I think this has something to do with the fact that we had both permits and the "flags" that are coded to trigger when X item is in action.
I too had the human permit and I was able to pass through the gate from both sides no problem multiple times. The gate just opened as soon as I approached with no interaction from the guards. I had even bought the house at that point, but they don't give the residence permit for it until you progress the story.
So as soon as I did progress the story and the residence permit showed up the next time I approached the gate the guard stopped me and asked to see my permit every time.
