Can I be ever sure my pawns are not sick?
Is there a way to be sure my pawns don’t have an early stage of dragon curse? Before they have red eyes and act weird?

Also can you catch it only via rift portal or can pawns wandering around the world also have it?

You can have your pawn get infected by fighting a drake/dragon or getting it from an already infected pawn, through the same means, by hiring them through the rift and resting at the inn one too many times. Your only visual are the red glowing eyes, if you see that definitely yeet them into the brine and hire yourself a new clean pawn. Your primary pawn if infected can be cured via tossing into the brine as well (brine meaning deep water).

You can't randomly catch it out of nowhere, as mentioned above. The Dragon Plague can only be caught from dragons, then can spread from an infected pawn via hiring through the rift. Pay attention and you should have no problems with it killing your town npcs or infecting your pawns.

Infected pawns will have red eyes. Quite easy to catch

Just talk to your Main Pawn before resting in your House / Inn.
Eyes will glow every 4 seconds, it's super noticeable.

Pawns wandering in the world absolutely can have the plague too, I often get accosted by them on the road, with pulsing red eyes and everything. Not sure if they can transfer the disease to your pawn(s) without hiring them tho.

Wandering pawns also have it.

However, it doesn’t tell me if I can be ever 100% sure if all my new pawns are 100% healthy.

just dont take pawns with a full face helm that can't have the visor raised so you can see the eyes, even if you do just un-equip the helm and give them some trash helm without a full face cover

You can never be 100% sure of that unless you pay atention to all the signs and causes and you are cautious. I'm now on my 3rd playthrough (NG++) and I never got infected pawns of wiped out towns.
Though, this plague annoys me and I didn't wanted to deal with it on my NG+ so I installed a mod that disables it completely because I wanted to focus on exploring and completition.

If are willing to use mods, there is a mod that displays the lvl of contamination of your pawns (1-10, at lvl7 they infect others) without disabling it.

I've been trying to give popular capcom pawns the dragon plague, mostly low level ones! :steamhappy:
You can have infected pawn with NO red eyes !

So your pawn can get the dragonplague from a seamingly normal pawn you recruited in the rift.
The red eyes is the last step before they trigger the dragon's plague if you rest in an Inn.

You don't trigger the dragon's plague if you rest in an inn if they don't have the red eyes.

For example, my first encounter with the dragon's plague in my game was when i recruited an infected pawn in the rift. Got the message (tutorial) right when i recruited him.
I then check he was perfectly normal, no red eyes etc.. and same for the other pawn.
Ended up in the Brine just in case anyway.

The only sign I got was my pawn saying he felt a surge of power in combat. Turns out that’s one of em cuz he just nuked vernworth. It was fun and nice to get to use that eternal wokestone finally.

The way I dealt with it... Inn save before you get the dragonplague popup... recruit someone, if yes, reload inn save... repeat every time you hire a new pawn lol. Also, I stop dragons/drakes from brainwashing the pawns by staggering them. Still keeping an eye out for any signs, but I've had no issues... I've even got the Eternal Wakestone... and made a few working forgeries at 30k each.
