Then I must've gotten really lucky. Or maybe it's because I was just observing it for a while and hiding behind a rock because of the scary, red beams coming out of the head, my pawn must've climbed it in the meantime and done the deed.
I tired it with lightning buff and I was not able to do it. I've seen videos of people doing it in one to two hits and I have never been able to replicate that outcome. The above method I posted is the only 100% surefire way to be Guaranteed a Preserved Medusa Head that I know of personally.
I guess it's different for everyone based on how you play I did it three times in a row with zero issues. I'm glad you got it to work though with lightning damage I may revisit this in the future. I still have to defeat the medusa 5 more times for the badge.
whatever the case and however random it may be i firmly believe lightning + slash is the key.

But it didn't work the first time, no idea why. I quit and loaded back in, tried again and 2nd time I opened up with the heavy attack on the head while it was down and then I started doing normal hits and it worked.
Seems like a bit of RNG.
I also had the damage reduction ring on. Otherwise you can burn through the first health bar pretty quick once she is down.
