the only way of telling in a rift is a very (very) subtle reddish/purple glow to the eyes, if the pawn has their eyes covered you won't see it. in game as it gets worse the glow intensifies and they're supposed to act different but it's easy to miss the acting out.
in short, I think nothing will happen if you do that as there's nothing special about it happening except your pawns will talk about it.
for it to have full impact you'd have to do 5 areas that are considered populated, including the first mining area.
it doesn't work that way unless every single time someone hired the pawn they never triggered it

well i cant get their eyes big enough and actually see whats happening in there
also im colorblind and it makes identifying certain shades of red very difficult
All in all a funny but pointless mechanic but it also has a "hidden" bonus, it fixes quest states and guard behavior. In my game i made the Batthal jail guards aggro and they wouldn't reset, getting Hugo out of prison also didn't progressed because of that. After the plague and respawn Hugo was out (instead dead) and guards were normal again.
I meann its a capcom pawn, they don't exactly check to cleanse them.
Its the fem human level 39 sphinxparent, you'll know it because it has a ton of hearts and thumbs up. of course a quest pawn got infected, and i last checked it yesterday so it there is a good chance she's still got it.
many of them are overnight. i was stuck when i blew up vern where i couldn't go to masquerade because my gear was in the inn who was dead. i needed to rez brant and sven to proceed, as they didn't respawn.
when i went to see the list it was 50-100 npcs dead, but a lot of vendors and quest related ones were overnight respawns
or pawns attacked me after being grabbed by a dragon
but that went away after i killed the pawn
I know what you meant. When they trigger the plague it goes away. So for that one to "always have the plague" means people are not triggering it... unlikely.

Just checked and in my game they don't have red eyes
The pawns attacking you is because the dragon has that ability, it's not the same. Your pawn being snippy / saying to shut up is definitely the plague.
it takes a while / days of resting, if you get it again. it has to progress to where they will not do a command when you tell them.
