Never encountered it since though - even after going back to try and find it.
Edit to say that I only ever encountered one from a quest and a random one in post game since then.
I didn't beat him and he despawned when daylight came, I should be able to fight him again I hope, right? Dude is STRONG.
in main game i fought him 2x, one at the substantial ruins around the path if you go east from vern and then north (running to beginning game) and then right before a bridge crossing over at the north east part of that path.
The mechanics of it are as follows, it appears after the place you just ran through. And killed a all of zombies\skelets there. Immediately on the point if you look at the online map he does not appear at once. I don't know why or how it works. Tested in a lot of places. But it's hard for me to explain every point where I met him. I did exactly that, went to one side killed monsters, reached the end, and when returning he appeared. where I was literally 5 minutes ago.

both at "early" night obviously (rested until night then run there). Both were there just as I was passing through. No killing or anything. There is always a dense fog all around the big area when he is up.
I did notice the large fog area as mentioned above, thought it was area specific but disapated after I killed him.
That said, anyone know if you can grab his head like Medusa? I duno it's just in his hands, I tried to run up and grab it but was a mage at the time so uI couldn't focus fire a weak point and try to knock it out of his hands like with blunt attacks or something.
Would be fun if we could kick it around like a football.
I fought one not near covered running water....but it was near running water....which ties back to the dullahan myth of not being able to cross water.

The time was around midnight. I did not encounter any ghosts. I did encounter some skeletons briefly before.
