For pawns you can have one main, that you can customise, throw to brazil, whatever, and 2 supports that are either capcom's or other players' . these can help you in finding your way toward quests or fight big monsters efficiently. Their AI, Learns from you and others.
Dragon's plague is real. If you notice your pawns saying strange things or not acting the way they normally do it might be a good time to chuck them into water to be swallowed by the brine. Dying seems to cure pawns of dragons plague.
When Pawns die in battle or from the brine they get battle scars which can only be healed in the holy waters of Battahl.
2. Dragonsplague is absolutely real: it's a disease pawns can contract while fighting drakes/dragons and it bears the risk of killing every NPC in the city you're resting while your pawn is infected. There's no definitive way of detecting it, look out for odd pawn behavior (annoyed comments and refusal to obey) and/or red glowing eyes. The only way to be sure you pawn is cured is to dismiss your hired pawns and brine (kill) your main pawn and then resummon it at a rift stone.
3. Don't eat yellow snow.
The first time you get dragonsplauge you will get a popup stating information about dragonsplague. It also takes quite a while to develop to a dangerous stage. The eyes of your pawn will start pulsing red more and more. Pawns also spit on the ground when they have it. They will also start talking about new things. Such as: "I feel better than ever" and "don't order me around, I know what to do"

The easy solution to this problem is to just throw your pawn into some water and let it die. You will have plenty of time to do so. It is also possible to pass on dragonsplague to another pawn by getting hired by another player. This will end your pawn's dragonsplague.

In early stages you are unlikely to see them but around 50 (which was when i first had the tutorial popup) you might. I met 3 infected, the first notification you get is pawn going "more stronger than usual" ish line or "feeling possessed by something larger" text.
Just keep in mind you can cure your main pawn by just throwing your pawn into water (brine).

1. One main pawn (yours) and two others (Other players or CAPCOM generated). For a total of 4-man party (including you)
2. It's real. You cure them (only your main pawn) by "forfeiting" them. Most efficient way to do it is throw them into a river. Others pawns cannot be cured. They would simply be sent back to their owners with dragonsplague. You can also just tell them to go away.
Where is this holdy waters of Battahl ?

It's weird cause there's a lot of overconfident/smug reactions from pawns by default, so I imagine it makes detecting the actual bad behavior slightly harder.
Interesting. I have a Kindhearted pawn and she never spits, unless infected. This probably means that pawns change their behavior towards what's considered a more abrasive inclination - which would be Straightforward.
spoiler : i'll say stuff i kind of noticed, and only became obvious after the border town was wiped off my pawn's eyes were becoming red little by little, until they were very noticeable behind her helmet. and she would say that she was stronger for some reason, and that she had too much energy or something like that.
