NPC's also have a 'life' they move and are only in their locations at specific times of day, just like the main quest you'll need to come at night to talk to the NPC, other NPC's may be in the morning or afternoon or maybe even in a different town.
As for NPC's vanishing while you are following them... no idea, possibly a bug, haven't run into it myself. It may also be part of the quest, if you can say what quest it is that may help.
Or (better option) Go to the riftstone and hire a pawn that has knowledge of the quest you currently have selected as a priority. Then using the 'Go' command will have them lead you where you need to go.
Basically in an effort to not waste time I had to go through my NPC log book, google the NPC’s name to figure out wtf was going on. Never had to do this for the first game.
I can take running around the field at a slow pace and whatnot but these quests are really aggravating.
I'm not sure what your life is like but rarely does someone give me vague instructions dab a yellow blob on a map and tell me to get on with it, most of the time people are pretty clear when they ask me to do something.
I have yet to summon someone out of the ether because they have done a task in an alternate reality IRL either.
Still, the majority of the quests will still put a big marker on your map you can follow, but some will definitely expect you to pay attention and make decisions.
If you are still stuck, recruit a pawn who know about the quest.
Every second you spend in a video game is a waste of time. Might as well just relax, take your time, and try to enjoy it. Some things shouldn't be rushed :)

I give them the exact location, time, t task, cost, materials and order of completion.
Your doctor doesn't say "To need an MRI buddy. Can't give you any more information than that I'm afraid."
He gives you the number of the office, the location, an appointment and a referral.
I honestly cannot think of even one real life example of vague instructions in the real world. Honestly not even one.
What real world corrolary do you have in mind here?
